Link to Care WA is powered by the Community Health Network of Washington (CHNW) – an affiliation of 21 community health centers that operate more than 140 clinics across Washington. CHNW’s goal is to get more people connected to health care by providing the tools and support they need to access and manage care online and by phone.

Link to Care WA improves health care access by making digital navigation resources available for Washington residents. Community Health Network of Washington created Link to Care WA with grant funding from the Washington State Department of Commerce. Link to Care WA supports Digital Navigation within Health Centers, a statewide hotline, and with community partnerships.
